Please note: When submitting the application, please prepare and attach the following documents:
- Certified copy of ID card;
- Certified copy of university degree;
- Certified copy of the certificate of completion of the professional exam;
- Description of work experience, on the form provided in Appendix 1C (PDF version of the document can be downloaded here);
- Employer’s letter attesting to relevant professional experience;
- Certified copy of the document issued by PIO/MIO stating that pension and retirement liabilities for all employees have been duly settled;
- Certified copy of the certificate of completion of the Training Programme / Development Programme (appendices 4A.1, 4B.1 and 4C.1);
- Proof of registration as a sole trader, additional or supporting business activity;
- Copy of the liability insurance contract or liability insurance policy;
- Certificate issued by the competent municipal court stating that the applicant does not have a criminal record;
- Evidence of payment of fees for the procedure.
